Egg Donor Application
There are number of convenient, secure, and confidential ways to apply to NAFG's egg donor program.

3. Supporting Materials
Your application is not complete without a copy of a photo ID and current
digital photos (see Photo Submission Guidelines). Digital photos should be
sent to inquiry@assistedfertility.com; copies of photo IDs may be sent by e-mail, fax,
or mail to the address above.
We are happy to accept scanned applications and other documents, as well as digital photos sent to inquiry@assistedfertility.com.
Whether submitted electronically, by fax, or by mail, all applications are confidential.
Full instructions and a description of the supporting materials required (these include a copy of a photo ID and recent digital photos) appear on the first page of the application.

According to the American Society for Reproductive Medicine's Ethics Committee guidelines published in 2000 and restated in 2007, egg donor compensation exceeding $10,000 is inappropriate. NAFG shares this view. Under no circumstances are donors paid more than $10,000. Donors are compensated for the time and effort required for the process, not for their eggs. NAFG offers the highest amount allowed by the ASRM in order to attract motivated, qualified egg donor candidates from around the country.
